Membership Directory Preferences

By checking the boxes below you may hide elements of your information in the Membership Directory.  Any box below left unchecked WILL be displayed in the Directory.  If you wish not to be listed at all, just check the first box, "Do NOT list me in the Membership Directory."  It is unnecessary to check the rest, the will be assumed to be hidden.

Professional Directory Information

The following information is only required if purchasing a listing the Professional Directory, which can be accessed from the myAHS dropdown menu.  Please be sure to include at least your city and state, and at least one contact method (phone, email, etc.). 

All members are welcome to provide this information if desired.
